If you need a copy of your 2006 W-2 form, please contact JAT Services website or call center as noted below. JAT’s website offers immediate access for viewing and printing W-2 forms for years 2002 – 2006. You may contact JAT’s call center to request that a copy of your W-2 be sent by mail.

For both the website and the call center, you will be asked to provide your Social Security Number (Login ID), Date of Birth (Password), and the Landry’s Pin # 1952 for verification purposes. Once logged in to the site you will be prompted to change your password from your birth date to a unique password of your choosing.
